RMC Opens Post-Acute Care Clinic to Support Patients After Hospital Discharge in Anniston

Anniston, AL Regional Medical Center (RMC) has opened a new Post-Acute Care Clinic (PACC) aimed at supporting patients during the critical period immediately following discharge from the hospital.

The Post-Acute Care Clinic is designed to provide short-term follow-up care for patients who may not be able to see their primary care provider promptly after leaving the hospital or who do not yet have an established provider. When needed, patients may receive coordinated follow-up care within 72 hours of discharge to help ensure a smoother transition from inpatient care to recovery at home.

“Healing doesn’t end when a patient leaves the hospital,” said Dr. Almena Free, Chief Medical Officer at Regional Medical Center. “The Post-Acute Care Clinic is an example of our commitment to patient-centered care, ensuring our patients feel supported, informed, and confident as they continue their recovery.”

During clinic visits, patients may receive a review of discharge instructions, medication education, and guidance to address early recovery concerns. The clinic functions as a transitional resource and is not intended to replace a patient’s primary care provider. Instead, staff work with patients to reconnect them with their existing physician or assist them in establishing ongoing primary care if needed.

The clinic also plays a role in care navigation, helping coordinate transitions to home health services, nursing facilities, or hospice care when appropriate. By collaborating with primary care providers and specialists, the clinic seeks to reduce the likelihood of complications and prevent avoidable hospital readmissions within 30 days of discharge.

The Post-Acute Care Clinic officially opened on January 12, 2026. It is located in the Professional Office Building, Suite 303, on the Regional Medical Center campus and operates Monday through Friday from 8:00 a.m. to 4:30 p.m.
